What We Provide
Physician Office Visits
Free medical appointments, both at our clinic and with volunteer providers in the community.
Dental Care
Free or up to 50% discount on dental services including cleanings, exams, and basic treatments.
Diagnostic Testing
Free lab work, X-rays, and other diagnostic tests through our volunteer network.
Pharmaceutical Assistance
Help getting the medications you need at low cost or free through assistance programs.
Information & Referral
Connections to specialists, community resources, and other services you may need.
Social Services
Care coordination and support navigating the healthcare system. Hablamos español.

How to Apply
If you think you are eligible to become a client of Health Care Network, please call us or visit us in person to register.
During the enrollment process, a staff member will interview you in person, which will take about an hour. You will be asked to provide:
- •Picture identification
- •Proof of income for the past 3 months
- •Proof of residence for the past 3 months
- •BadgerCare (Medicaid) denial letter
A Word About Our Fees
There is no charge for the services you receive at Health Care Network. However, there may be charges for other office visits, including surgery, injections, prescriptions, certain medical procedures, and dental services. Please ask a Health Care Network staff member to inform you of any fees.
