At Health Care Network, we’re always looking for new ways to meet patients where they are — in heart, culture, and community. This year, we’re excited to share the launch of a new diabetes management program designed specifically to reflect the cultural values and daily realities of the people we serve.

The program is being led by Bridget Diaz, our Doctor of Nursing Practice (DNP) candidate in Psychiatry from UW-Milwaukee, in partnership with three of our dedicated Health Care Coordinators — Esmeralda Muñoz, Marcela Maldonado, and Veronica Meltzer. All three recently completed certification as Community Health Workers, giving them new tools to better connect, educate, and empower patients.

The program takes a more personal and culturally aware approach offering:

  • Monthly in-person education and support sessions
  • Weekly follow-up calls for encouragement and accountability
  • Culturally tailored meal ideas that honor traditional foods
  • Help connecting with community resources
  • Support for tracking physical activity and blood sugar
  • Regular mental health check-ins
