Racine County Community Clinic, Inc. officially opens in basement office space donated by First Presbyterian Church. Office hours: Tuesday & Thursday, 10am–3pm.

Barb Tylenda hired for part-time Executive Director position. 1,090 appointments scheduled since inception.

Executive Director position becomes full-time. On-site services initiated with 6 retired MDs and 7 RNs. Total 2,200 appointments scheduled to date.

Agency moves to 904 State Street thanks to State Grant and successful Capital Campaign. Health Care Network honored by President Bush as Point of Light. Becomes member of Racine Area United Way.

Executive Director Barb Tylenda selected as the YWCA Woman of Distinction for Public Service.

Surgeon General David Satcher recognizes volunteers. Dr. William Little quoted in U.S. News & World Report. 68,181 appointments provided since inception.

On-site dental clinic opens April 6, 2005. 30% increase in dental services in first quarter. By year end: 12 dentists, 7 hygienists, 7 assistants volunteering.

Greater Racine Mammogram Coalition formed. Agency receives Outstanding Service Award from Racine Interfaith Coalition.

Monroe Thomas receives Volunteer of the Year Award. Barb Tylenda receives Madonna Medal for Professional Excellence. Record prescription assistance: 6,326.

Mayor and County Executive designate October 18, 2012 as Health Care Network Day. 173,511 appointments provided by volunteers to date.

Alison Sergio becomes Executive Director. 4 staff become Certified Application Counselors for ACA. 229 patients assisted with BadgerCare+, 44 with Marketplace plans.

Becomes partner of WAFCC, awarded Bronze Seal of Excellence. AmeriCorps VISTA member program begins.

Social services officially begin with on-staff social worker funded by Wheaton Franciscan Sisters Ministry Fund. Building A Healthier Community Capital Campaign kicks off.

Dr. William Little featured in UW School of Medicine Quarterly Publication after 30+ years with HCN. Earned Gold Seal of Excellence from WAFCC and Gold Standards Rating from NAFC.

Self-Monitoring Blood Pressure program launches. Patients receive hypertension education and self-monitor at home before appointments.

FHASES electronic records system implementation begins. 200+ paper charts scanned and uploaded in first year. CPT codes added to quantify total healthcare value provided.

Partnership with Sostento begins, offering free Uber rides to enrolled patients for improved healthcare access.
